Abstract

Introduction: uterine sarcomas are malignant mesenchymal neoplasms with a low incidence, but extremely aggressive and poor prognosis. Uterine leiomyosarcoma is a type of uterine wall sarcoma with a high mortality rate and is the second most common uterine sarcoma.

Case presentation: female patient, 65 years old, with a history of high blood pressure, no known toxic habits. Gynecological history of menarche at age 12 and last menstruation at age 52. Obstetric history of three pregnancies with two caesarean deliveries and one induced abortion. She came to the consultation due to presenting metrorrhagia approximately one month ago, leucorrhoea and pelvic pain. Physical examination reveals a polypoid, easily bleeding, whitish and leukokeratotic lesion in the cervical canal between hours 12 and 3 of the clock. Based on the signs observed and the symptoms reported by the patient, a presumptive diagnosis of cervical or endometrial neoplasia is reached. It is decided to order a biopsy of the lesion found and schedule an appointment for follow-up.

Conclusions: due to the advanced stage of the disease, the patient was not eligible for surgical treatment; biopsy was the most important complementary examination in the diagnosis of leiomyosarcoma. The use of radiotherapy and chemotherapy allowed for timely treatment of the tumor. The patient is currently undergoing medical consultation.
